Certificate,
Reading Records and Report Card Labels
The following certificate and reading records were designed
by the Youth Services Department. These are not the Upstart products designed
by CSLP. MHLS will not be printing any of these items for distribution but
we encourage you to make as many copies in your own library as you like. All
three items are in PDF format.
Reading Certificate - The Reading Certificate was designed to be printed on
an 8 1/2 x 11 (Letter size) piece of paper.
- Once the form has opened in PDF, choose Print. Click OK to Print.
If you have trouble printing make sure that on the Print menu under Page Handling,
(use the drop down menu for Page Scaling) select Fit to paper, then check
the box labeled "Auto-Rotate and Center" and also the box labeled
"Choose Paper Source by PDF page size." Click OK to Print.
Reading Record (Title), Reading Record (Time)
- There are two types of Reading Records: The first, Reading Record (Title),
has one column for Title and Author to keep track of books read. The second,
Reading Record (Time), has a column for Title and Author and also a column
for Time Spent Reading. The Reading Records (there are two pages for each
type of record) were designed to be printed on an 8 1/2 x 14 (Legal size)
piece of paper, although you can shrink the page to 8 1/2 x 11 (Letter size)
if you do not have Legal size paper.
- To print either document on 8 1/2 x 14 (Legal size)paper, choose
Print, once the document has opened in PDF. On the Print menu under Page Handling,
check the box marked "Choose Paper Source by PDF page size." The
drop down menu for Page Scaling should be set to None.
If you have to manually feed the paper to your printer, choose Properties
on the Print menu and under Paper / Quality use the drop down menu for Paper
Source and select Manual Paper Feed, then click OK. This will return you to
the Print menu; click OK to print.
- To print either document on 8 1/2 x 11 paper, choose Print once the
document has opened in PDF. On the Print menu under Page Handling use the
drop down menu for Page Scaling and select Fit to paper, and then check the
box for "Auto-Rotate and Center." Make sure the box for "Choose
Paper Source by PDF page size" is not checked. Click OK to Print.
The Reading Records were designed to be folded into a book. Make page one and two into a double-sided document on the copier, fold and go.
Teen Reading Record - This record was designed to be printed on 8 1/2 x 14 (Legal size) paper. There are four records to a page. They can fit in their back pocket or in a paperback. There is a rating system next to the Title, Author section so you know what they liked and what they didn't.
- To print on 8 1/2 x 14 (Legal size) paper, choose Print, once the document has opened in PDF. On the Print menu under Page Handling, check the box marked "Choose Paper Source by PDF page size." The drop down menu for Page Scaling should be set to None.
If you have to manually feed the paper to your printer, choose Properties on the Print menu and under Paper / Quality use the drop down menu for Paper Source and select Manual Paper Feed, then click OK. This will return you to the Print menu; click OK to print.
For a double sided version print two copies of the page, make a double sided document on your copier, cut and distribute.
Report
Card Labels in Black and White, Report
Card Labels in Color - This idea was generously donated by RCLS. You will
have to make contact with your local school to see if they would be willing
to affix the labels to the report cards and if so, how many you would have
to provide. These labels are in a word document and meant to be used with
Avery 5163 Labels (saved as a Rich Text Format).
- Choose color or black and white. Click on the appropriate link and
once open, go to File and choose Save As. Save the document in a convenient
location by using the drop down menu.
- Insert the 5163 Avery labels into your printer and print as many
copies as you like.
- You can also print them on a regular piece of paper and copy them
onto the labels through your copy machine.
