Summer Reading Program Final Report

In 2004 the Mid-Hudson Library System created an online version of the
NYS Summer Reading Program Final Report.
Due to the great success of this form, all SRP Final Reports are now submitted online.

Directions: Please read the directions below before filling out the report.

• Click on the link below to access the Final Report Form.
• After completing the online form click submit form. The next screen will have all of your information and it will read across the top "Required: Please confirm your submission." Please print a copy of this page for your records. Make sure all of your information prints out before continuing.
• After printing a copy of this page, scroll to the bottom and click "I confirm that this information is correct." Your report will not be submitted unless you click on this button.
• You may be prompted at the next screen to enter letters from a graphic into a text box for authentication purposes; please do so or your report will not be submitted.

If you find it helpful to write out all of your information before you fill out the online form, you can print out a paper copy of the final report here (PDF). Please remember that the final report you submit to MHLS must be done through the online form. Don't forget to keep a copy for your records. Thanks.
