Summer Reading Program Final Report

In 2004 the Mid-Hudson Library System created an online version of the
NYS Summer Reading Program Final Report.
Due to the great success of this form, all SRP Final Reports are now submitted online.

• Please read all questions thoroughly before answering.
• If a question does not apply to your library, leave it blank.
• The last section of the report has a button that reads "Click here to submit this report." When your report has been successfully submitted you will be sent to the "congratulations" webpage.
• A PDF version of your completed report will be sent to you via email within 1 to 2 business days.
